Moving the pole with your arms, and the plant in general

I am a second year vaulter, and my plant is in need of much work. I have been doing drills for several weeks now with very little progress, so I was wondering if any of you had any tips on drills and mental techniques to correct the plant. When I plant, my left arm generally collapses, my head looks up, and my hips immediately swing through, generally with the trail leg not very straight. I can still bend the pole and get in, but based only on runway speed. Here is a picture of my plant:

How do you get yourself to keep your hips back and move the pole forward with your arms? Thanks!

Try and push your hands up at the bar when you plant
