one time this happened in the high jump in my sectional track meet my senior year in high school.
the jumper had already won the meet and he was a really good jumper(a 6'6" pr i believe)
he had the bar raised to 6-7
then he had a really good attepmt but he hit the bar a little and it bounced off of the pegs and on to the top of the standards.
the officials, not knowing the rule or what to do credited him with a make and then decided to measure the bar.
the result was that he was not only credited with a make, they gave him a make at 6'9.5" when the correct ruling is a miss at 6'7"
i could not believe that he was given that mark.
