My first thoughts are that your run is off balance (you're leaning way forward), and that this combined with a very over-gripping bottom hand (your hand is on top of the pole rather than supporting it from underneath) lead to a late plant and a take-off position 18"-2' under. The pole should not be bent before you leave the ground!!!

I dont quite understand your "over gripping bottom hand reference". Can you explain a little more in depth what that means please?

It was definitely a little vague...

Basically, you don't want your palm facing the ground if you're carrying the pole parallel to the ground. It should face almost entirely out to the right. Does this make sense? I'll find pictures if you need them.

Carrying the pole correctly relieves a lot of stress in the arms and back. It helps your run, plant, and take-off directly, and so it helps everything else indirectly as well.

Please comment on my form with constructive criticism! Can we please replace the archaic term FORM with the word TECHNIQUE. :)
