Letting go too soon.
On all my vaults, I am coming off the pole too soon. What drills, mental barriers could I do to get myself to stay on the pole longer so I can increase my height?

Also, try jumping at really high bungees. Whatever the highest bar you can clear on a regular basis is, start a bungee a foot higher, concentrating on staying in your vault until your hips get to it. Each jump, raise it 6" and keep trying to line up with the top of the pole and get your ankles over it until you eventually can't reach it anymore. A general rule of thumb is that whatever the highest bungee is that you can snag with your ankles, you can clear a bar 2'6" below that. For example: I have a girl who can snag a 15'6" bungee and can clear 13'. The high bungee forces you to have to stay in your vault longer. Then when you get to that highest bungee that's just out of reach, for fun, put up a bar 3' below it but keep going for the bungee - great way to push yourself - you should clear the bar if you continue to line up for the bungee and you'll go in between.
