Critique Me Please

So for the past 9 months i have been out with a rotator cuff injury. I came back earlier this week had one practice session and then had this meet. I was running a 4 step from 51' holding about 11'5-11'8 on a 14 135 pole. The bar is either 10'6 or 11 i am not sure. I plan on moving back to a 5 or six step this week so would appreciate any advice while I am still on a short approach. I felt like i did not drop my shoulders enough which led to me shooting outward (that may have also been because of the mostly straight poling). http://www.youtube.com/watch?v=ji2ijci1 ... f7t1JD6s_M

That tells the whole story old son -any advice you get will be black magic coaching! Come back with some vaults after three months training -at least!
