problem with run and dropping the pole

This is my 2nd season vaulting. last year as a freshman i cleared 9 feet. this year i cant seem to go any higher. my coach says that my problem is with the approch. he says that i seem to slow down on the last two steps, that my timing with planting the pole is horrible, and that i pull in on the pole while im in the air. im 5'8" 140 lbs and using a 13' 140 spirit pole. i have noticed that i some times i stall in the air or i leave the ground before the pole is in the box. this is starting to become dangerous. once i actually spun around on the pole 4 times before landing on the front sections of the pits. that was scarry s***.another time the pole actually bounced out of the box while i was in the air and landed in the pits.
my questions are: what should i be doing differently? how can i get the timing down better? what should i do so i dont loose speed at the end of my run? are there any books or videos that you would recommend?
