pushing off the pole

Honestly, I would get on a smaller length pole and work on vaulting above your hand hold. You are probably coming down on your stomach because you aren't driving the pole to vertical and staying in the flag postion to reach out to the cross bar. Get on a shorter pole and it will help you to get inverted and drive the pole to vertical quicker. Also practice with standards all the way back. when you take off concetrate on feeling like your trail leg is nailed to the runway so that your swing will stay long and generate momentum. But with out seeing it, this could all be wrong.

Without seeing video of you, its hard to determine exactly what you need to fix in order for you to swing completely to vertical. Any number of things can cause a vaulter to not swing and flag out over the bar like you seem to be doing. The advice some are giving to jump on shorter poles from a shorter run is good, but without knowing what you are specifically doing in your vault, we can't help you fix your vault.
